Nalmefene HCl Administration

FLEXIBILITY To be given as iv, im, or sc injection1
  • Primary administration route for Nalmefene HCl is IV injection
  • If venous access cannot be established, Nalmefene HCl may be given as IM or SC. It is not intended or indicated for intranasal use with a nasal atomizer

RECOMMENDED NALMEFENE HCl INJECTION ADMINISTRATION

  • Like all drugs in its class, Nalmefene HCl is not the primary treatment for ventilatory failure. In most emergency settings, treatment with Nalmefene HCl injection should follow, not precede, the establishing of a patent airway, ventilatory assistance, administration of oxygen, and establishment of circulatory access1
  • While the Nalmefene HCl Full Prescribing Information references dosing based on 70 kg body weight, clinical efficacy and safety studies conducted with Revex administered doses of Nalmefene without regard to body weight2

RECOMMENDED DOSING FOR PATIENTS

Patients without opioid dependency1

INITIAL DOSE
0.5 mg / 70 kg
SECOND DOSE
1.0 mg / 70 kg

2–5 minutes later, if needed

Patients with
known or suspected opioid dependency1

CHALLENGE DOSE
0.1 mg / 70 kg

If there is no evidence of withdrawal in 2 minutes, proceed with the initial dose

INITIAL DOSE
0.5 mg / 70 kg
SECOND DOSE
1.0 mg / 70 kg

2–5 minutes later, if needed

Recommended Dosing1

If a total dose of 1.5 mg/70 kg has been administered without clinical response, additional Nalmefene HCl injection is unlikely to have an effect.

Patients should not be given more Nalmefene HCl injection than is required to restore normal respiratory rate to minimize potential cardiovascular stress and precipitated withdrawal symptoms.

Nalmefene elimination half-life is based on pharmacokinetic data. The mean terminal elimination half-life is 10.8 hours and 9.4 hours in younger (19 to 32 years) and elderly (62 to 80 years) healthy male subjects, respectively, following a Nalmefene HCl 1 mg IV dose.1
